First off, this form is more along the lines of how I would like mine to look but so far, unable to get CLOSE to this. Detailed instructions would be a big plus....please assist or guide me to the instructions page...

My second thought is when a user goes in to check the status of tickets, the image above is what they see. Is there a way to just get a list?
